South Main Clinic Awarded Cambia Foundation Grant To Build Health Equity

The South Main Clinic at 麻豆学生精品版 was awarded funding from Cambia Health to support innovative whole-person care strategies focused on eliminating disparities. A grant award of $100,000 will support the addition of a "Care Collaborator" to South Main Clinic's team, with the goal to link new patients to medical care and other services.

South Main Clinic has provided family care for underserved populations for nearly 25 years. Last year, the Clinic served over 5,580 patients (17,300 visits) who speak over 39 different languages. Through multi- disciplinary care teams and partnerships, South Main provides access to critical pediatric, prenatal, primary, and dental care. Wrap-around services from government and community partners complement the clinic's direct clinical care, offering families a supportive, convenient, and unique care experience.

The clinic's many community partners include the Division of Workforce Services, Salt Lake County Library, Division of Child & Family Services, Utah Department of Health, Reach Out and Read, Bell Helmets, Utah Legal Services, Utah State University Center for People with Disabilities, Family Voices, Utah Parent Center, Utah Leadership Education in Neurodevelopmental Disabilities, Salt Lake County Health Department, and Holy Cross Ministries.

The new Care Collaboration model will enhance these efforts by connecting patients to all of the services that South Main and its partners have to offer, addressing the key determinants of health: physical, spiritual, behavioral, emotional, social, and financial.

In total, Cambia Health announced 14 new investments in organizations who share the Foundation's desire to eliminate disparities by focusing on the whole person: their physical, emotional, social, financial, and behavioral health.
